Conclusion of an Individual Medical Support agreement.
Upon payment for the service through the form on the website or using the details, an invoice is sent to the email address specified during registration.
This invoice is a written offer (offer) from the Contractor to the Customer to conclude
a Service Agreement in accordance with the procedure provided for in Articles 432-444 of the Civil Code of the Russian Federation (Civil Code of the Russian Federation). Service agreement
it is concluded by acceptance (acceptance) of the offer by the Customer on the terms stipulated in this offer, which is considered compliance
with the written form of the Contract (clause 3 of art.434, clause 3 of art. 438 of the Civil Code of the Russian Federation)

1. The subject of the Contract is the provision by the Contractor to the Customer of the services listed in the Invoice.
2. The remuneration of the Contractor under the Service Agreement is the total amount indicated in the Invoice.
3. An essential condition for concluding a Service Agreement is the Customer's full one-time payment of this Invoice within 5 (five) business days from the date of issue indicated therein (the deadline for acceptance of the offer). Such payment will be considered the only possible proper acceptance of this offer (Clause 3, Article 438 of the Civil Code of the Russian Federation).
4. Services under this Agreement are provided no later than 30 calendar days from the date of payment of this Invoice.
5. After the completion of the provision of services, the Contractor may send to the Customer an act of acceptance and transfer of the services provided (hereinafter referred to as the "Act"). The Customer undertakes to sign the Act and hand over the second copy of the Act to the Contractor within 3 (three) business days from the date of its
receipt. If the submitted Act is not signed by the Customer within the specified period and/or its second copy is not returned to the contractor, such Act is considered approved by the Customer, and the services are rendered qualitatively and in full.
6. The contractor has the right to involve the partner company EMPORICUS LLC (TIN 972929279) in the provision of services under this agreement https://emporicus.ru /. The Contractor is responsible to the Customer for the consequences of non-fulfillment or improper fulfillment by the involved third parties of their obligations.
The subject of the agreement.
1. The Contractor, on behalf of the Customer, undertakes to provide non-medical services of individual
medical support to clients related to the organization of the highest quality medical care and related services in the amount, manner and on the terms provided for in this Agreement.:
1.1.1. Organizational assistance in choosing a medical or service organization from the List of the Ministry of Defense and
personal support (supervision) in the process of receiving medical consultations and undergoing non-hospital examinations of Clients (non-hospital support),
including the following:
- appointment of a personal medical coordinator for the Client, as well as the possibility
of contacting the call center daily, from 09:00 to 21:00 on a dedicated line 8 800 250 62 03
-free in Russia);
- providing the Client with additional information about the activities of various specialized and
multidisciplinary medical institutions (selective selection and systematization of this information based
on partnerships with medical institutions and publicly available information about the Ministry of Health) from the List of medical institutions;
- organizational assistance to the Customer in obtaining consultations with the necessary specialist doctors, as well
as medical examinations in the Ministry of Defense closest to the Client's place of residence (work), or at his request (from the List of Ministries of Defense);
- tracking the results of the provision of services to the Client and, if necessary, organizational assistance in
additional examinations or obtaining a "second opinion";
- providing other assistance (accompanied by a personal medical coordinator of the Client in the Ministry of Defense) in
the process of receiving medical consultations and undergoing non-hospital examinations (if necessary).
1.1.2. Organizational assistance during hospitalization (hospital support) Clients, which includes the
following:
- providing the Client with additional information about the activities of various specialized and
multidisciplinary medical institutions (selective selection and systematization of this information based
on partnerships with medical institutions and publicly available information) from the List of medical institutions;
- organizational assistance in carrying out hospitalization to the recommended medical institution based
on partnerships with medical institutions, applicable laws, by-laws and other regulatory documents; in this case, if inpatient treatment for the selected profile is organized within the framework of the compulsory medical insurance system, organizational assistance is provided for the purpose of providing individual medical services, determining the conditions of placement or provision services based on the individual needs of the Client;
- the constant presence of a personal medical coordinator in touch (and, if necessary, personal
presence in the Ministry of Defense) with the Client during the entire period of hospitalization in order to monitor the quality of services and
provide additional organizational assistance, if necessary.
1.2. The services specified in clauses 1.1.-1.2. of this Agreement are provided to no more than 1 Client. If the
number of Clients served by the Contractor at the same time exceeds the above limit, then each Client above the specified limit is paid separately by agreement of the Parties.
1.3. If the Contractor needs to provide other services or exceeds the volume
of services specified in this Agreement, the Parties have the right to conclude an additional agreement to this Agreement.
The agreement. The cost of such services will be determined by agreement of the Parties.
